Will and mandate research request form
To fill out the form on screen
Prerequisite: To fill out the research request form in PDF format, you must have Adobe Reader (version 8.1 or more). You must then print the form and forward it to the Chambre des notaires.
Instructions: Download and save the PDF file on your computer. If you have the Professional Adobe Acrobat version, you will be able to complete the form on screen, save then print and forward it to the Chambre des notaires.
On the other hand, if you have the free version of Acrobat, Acrobat Reader, you will be able to fill form on the screen, but you will not be able to save your data (when you close Acrobat Reader, your data is erased). Your only option is to complete the form, print it before closing Acrobat Reader and then to forward a copy to the Chambre des notaires.
If you experience difficulties to fill out the form on screen or do not have the necessary prerequisite, download, then print the form in PDF format, complete it by hand and forward it to the Chambre des notaires.
